The Coiled Altar is boss seven of eight in the Venomous Abyss and the last one before Ula'tek. A Heroic kill puts 315 gear at Hero 4/6 on the table, Normal puts 302 at Champion 4/6.
Two of its drops are flagged Very Rare and do not appear anywhere else in the tier. Aman'muso, Warlord's Vengeance is a one-hand axe and Maze-roa, Warlord's Fury is the two-hander. Both are cantrip weapons carrying a Venomfang effect on top of their stats.

Why this kill is worth buying rather than pugging

The encounter runs three stages plus an intermission and it carries more named assignments than anything else in the raid short of the final boss. Orb collectors in phase one. Two alternating Guillotine soak groups of at least five players each on Heroic. Fixated players walking their Manifestations of Dread to a stack point in phase two. Tanks taunt swapping after every frontal. Interrupters on Wail of Terror. Everyone soaking Fragments during the intermission without dropping the raid too low to survive the next one.
Phase three then runs every mechanic from stages one and two simultaneously while requiring both bosses to die together, because killing one enrages the other for 500%.

Normal or Heroic
Normal pays 302 at Champion 4/6 and gives you the encounter with the full phase three requirement but none of the Heroic layers, so no two-group Guillotine split and no Spiteful Soulcoiler adds.
Heroic pays 315 at Hero 4/6 and puts a 318 Myth 1/6 item in your Great Vault. It is also the difficulty that counts toward Ahead of the Curve, since AOTC requires Ula'tek on Heroic and Ula'tek requires this boss dead first.

Worth knowing before you book

Blizzard hotfixed this encounter on four consecutive days between September 1 and September 4, reducing Coalesced Venom, Venom Rupture, Volatile Venom, the Defilement healing absorb and Soulcoiler health, with the largest cuts going to smaller raid sizes. It is a softer fight now than it was in August.
