Each hit with Boneshatter generates a Trauma stack. Each stack adds physical damage to your attacks but also deals physical damage back to you every time you swing. Stacks last 6 seconds and reset at a certain threshold. The Slayer's life overleech and physical mitigation from gear and passives keep you alive through the self-damage while your actual damage output keeps climbing the longer you fight.
