Three ways, none of them reliable. It is a world drop with no dedicated boss or league mechanic behind it, it can be chanced from a white Heavy Belt with Orbs of Chance, or it can be bought from another player. There is no method that lets you farm toward it deliberately.

Killing a Rare monster gives you its modifiers for 60 seconds. Those modifiers stack as you kill more, so the belt gets stronger the denser the content is, which is why it is the premier mapping and clear speed item in the game.

Level 50. It is a Heavy Belt base, so it also carries the stun threshold bonus that base provides alongside its unique modifier.

They solve different problems. Headhunter is offence and speed, Mageblood is defence and consistency. On a squishy build Headhunter can actually get you killed, because it gives you nothing to survive with while encouraging you to dive into packs.
