Reach Season Pass Level 69 in the Into the Dark season and claim it from the reward track. The season is time-limited, so you need to hit Level 69 before the season ends.

Not currently. The Caduceus is exclusive to the Season Pass reward track. After the season ends, it may be added to the general loot pool, but for now, the pass is the only source.
