They are Season 15 encounters that rebuild locations, bosses and enemies from earlier Diablo games inside Diablo 4. They come in three sizes: short open-world fights that appear while you explore, full-length dungeons with fixed entrances, and multi-level Capstone dungeons built around the Prime Evil showdowns.

By clearing one of the largest Waking Nightmares and taking a different Splinter from it. You carry only one at a time, and swapping does not wipe the reputation you have already built on the track you are leaving, so nothing is lost by changing alignment mid-season.

Torment 10 and above. Below that the Capstone nightmares still run, just not in their Pinnacle form. The Pinnacle versions carry the higher risk and the better rewards, and their difficulty is fixed rather than scaled to you.

They are the season's main progression, not a side activity. Objectives, Splinter access and a large share of the Season 15 loot run through them, and the Capstone tier is where the rewards concentrate.
