Primal Nether is the gating reagent for endgame BoE crafted gear in TBC. Belt of Blasting, Belt of the Black Eagle, Cataclysm Boots, Spellstrike Hood and Pants and similar craftables all require Primal Nethers in their recipes. Most burn 2 to 3 per craft. They are also tradeable as of Patch 2.4, so you can sell or buy them on the auction house if needed.

Guaranteed drop from the final boss of every TBC Heroic 5-man dungeon. They have a low chance to drop from the final boss of regular level 68+ dungeons too, but Heroic is the reliable source.

Yes. Honored reputation with Honor Hold or Thrallmar, Cenarion Expedition, Lower City, The Sha'tar and Keepers of Time is required for the Heroic keys. If your account is missing any of these, the All Heroic Dungeons Attunements add-on covers the rep grinds and key acquisition.
